Patient Outcome Collection

Edited

What are clinical outcomes?

Clinical outcomes are the changes in the health status of individuals or populations that result from specific healthcare interventions, treatments, or procedures. These outcomes are typically assessed and measured to determine the effectiveness of medical care and to guide decisions about patient management.

Clinical outcomes can be objective or subjective and may include a wide range of parameters.

Evaluation Criteria

The following were used to evaluate all outcome metrics:

  • Falls inside the dietitian scope of practice and primary interventions for the specialties dietitians treat

  • Can be improved by nutrition intervention

  • Actionable within a 6-month period to allow patients and dietitians to detect signals and adjust accordingly

  • Can be accurately captured, and/or has the potential for expansion of capture with future partnerships like Apple HealthKit, laboratory and other diagnostic companies

  • Derived from a validated questionnaire with little or no modification

Please keep in mind that outcomes are collected over time and are only one piece of the puzzle; you'll want to collect additional data to deliver outstanding care to patients.

Where do we collect outcomes?

Outcomes are collected in three ways:

  1. Entered by the provider in the patient chart note (either in the labs and tests, lifestyle, questionnaires, or medications section)

  2. Entered by the provider in the patient's profile within the Provider Portal

  3. Entered by the patient directly in the Patient Portal or app

How often do we collect outcomes?

You should collect new patients outcomes every 30 days, or when new outcomes are available. You should not enter an outcome that has already been collected (for example, you should only input new weight information).

Patients and providers will be prompted to collect outcomes if no new information has been shared in the last 30 days.

Viewing Patient Outcomes

Providers can view patient outcomes directly on a patient's profile within the Provider Portal. To do so, navigate to the Provider Portal, and then select the patient's profile (either through the patients tab, your home page, or directly from a chart note).

You can scroll up and down to see additional results, and then toggle left and right within a category to see a different question within in outcome category.

Patients will have the opportunity to view their outcomes in a similar way in the Patient Portal / app. The one difference in the provider vs. patient view are weight graphs, which are only shown to patients who do not have ED, any ED subspecialties, or disordered eating selected as a specialty upon signup, and a condition thereafter.
