Overview

The Nourish Program provides personalized nutritional counseling to patients across more than 20 specialties, all delivered via telehealth and covered by health insurance. 94% of patients pay $0 for sessions!

Patients typically begin with weekly, 55-minute sessions with a registered dietitian. While the expectation is to maintain weekly appointments for the first 8 weeks, session frequency can be adjusted based on clinical judgment, patient progress, and individual preferences.

The total length of the program varies depending on the dietitian's care plan and the patient's progression, but typically, optimal clinical results are observed after 12 sessions.

Throughout the Nourish Program, both patients and dietitians have access to a suite of tech tools - including meal logging, provider messaging, and outcomes collection - designed to simplify behavior tracking and facilitate the achievement of health goals. These features are seamlessly integrated into the Nourish platform and app, providing a robust support system for patients on their health journey.


Program Steps

  1. Sign-up & Intake

    • Patient Matching: Interested patients visit the Nourish website and complete a sign-up flow where they are matched with a registered dietitian based on their stated health goals, preferred counseling style, insurance coverage, state licensing regulations, and more.

    • Intake forms: Before the first session, patients are prompted to fill out a comprehensive nutrition intake form which collects information on their medical history and specific health objectives. This enables the dietitian to tailor the first session and develop a personalized care plan. Patients will also be asked to sign Release of Information (ROI) forms for external care team members to allow for care coordination, and the Nourish Info, Consent, Release & HIPAA agreement.

    • Welcome Email: An automated welcome email is sent to the patient prior to their initial session, introducing their dietitian and providing an overview of the Nourish Program.

  2. Initial Session

    • Build rapport: Take time at the beginning of the session to share a bit about yourself and ask about the patient's interests or experiences. Be intentional about building rapport from the start, as a strong rapport correlates to a positive patient experience, stronger retention, and better clinical outcomes.

    • Comprehensive Nutrition Assessment: In the initial session, the dietitian conducts a comprehensive nutrition assessment to understand the patient’s lifestyle, medical history, and nutritional needs. This allows the dietitian to assess the patient’s nutritional needs, understand where the patient is at in their care journey, and develop a personalized treatment plan to help the patient reach their health goals.

    • Establish expectations: The dietitian will educate the patient on Nourish’s Clinical Philosophy and set expectations for the Nourish Program, including weekly, recurring sessions.

    • Set goals: Together, the dietitian and patient will establish both short and long-term goals. The dietitian is expected to provide actionable recommendations and resources.

    • Schedule follow-ups: The session will conclude with the dietitian scheduling follow-ups with the patient. Best practice is to schedule weekly, recurring sessions for the duration of the Nourish program (typically, 12 weeks), but the expectation for the dietitian is to hold weekly sessions for at least eight weeks.

  3. Follow-Up Sessions

    • Continue to build rapport: Check-in with your patient. What happened since the last session? On a personal level, did they do anything fun? On a professional level, how did they progress towards their short-term goals? Regardless of success, demonstrate active listening, show empathy, and offer support as needed.

    • Implement treatment plan: In follow-up sessions, the dietitian will work with the patient to fine-tune and implement the personalized treatment plan. This includes monitoring progress, collecting outcomes, updating short- and long-term goals, and providing actionable education, recommendations, and resources that will help the patient build sustainable habits.

    • Schedule follow-ups: Each session should conclude with the dietitian scheduling follow-up sessions. Follow-ups should be scheduled weekly for at least the first 8 weeks. After 8 weeks, dietitians should consider their own clinical judgement and patient needs/preferences to determine the ideal session frequency.

  4. Between Session Support

    • Leverage the Nourish app: Between sessions, the dietitian and patient will leverage the Nourish app to support the patient's progress toward their health goals. Features include automated appointment summaries, resource with action items and curated resources, goal tracking, 1:1 messaging, meal logging, and more.

    • Set expectations: As dietitians are paid on a per session basis and not compensated for time spent outside of the session, dietitians should set expectations with patients regarding their preferred availability / engagement outside of session. It is expected that dietitians check messages each day they see patients. Even brief engagement outside of session, such as reacting to a meal log, improves the patient experience and builds rapport.

  5. Graduation

    • The natural outcome of effective, clinical-grade medical nutrition therapy is patient graduation.

    • Graduating from care does not mean that patient engagement with Nourish ends! Graduated patients should be encouraged to continue using the Nourish app.

    • Patients may benefit from monthly or quarterly check-ins after graduating. Use your clinical judgement to determine the appropriate session frequency for offering accountability or making iterations on the treatment plan.
