Overview

The vast majority of Nourish patients are matched with a dietitian and schedule their initial session via the sign-up flow embedded within our website, usenourish.com.

Using data collected in the sign-up flow, our scheduling algorithm surfaces the best matching providers with the most initial appointment availability to the patient. The patient can then browse provider profiles to select their preferred dietitian and their desired initial session date and time. When patients schedule a session with you, you will be notified via email and the session will automatically be added to your Provider Portal and Healthie calendar.


Patient Intake Forms

After the patient has scheduled the initial session via the sign-up flow, they will be prompted to complete the following intake forms. Please take a moment to review the table below, outlining the types of forms presented and which are required.

Purpose

Nourish Info, Consent, HIPAA, & Release Agreement

Required. If this form is not completed, the session should be canceled, and the patient reminded to complete before scheduling again.

Provides education and obtains patient consent for privacy & HIPAA agreements.

Nutrition Intake Form

Optional - If incomplete prior to initial session, no longer required.

Collects medical history for dietitian to review prior to initial session.

Release of Information

Optional, but highly encouraged for insurance/billing purposes - If incomplete, ask patient to complete in initial session

Allows Nourish to communicate with:

  • External care team and/or family to coordinate care

  • Insurance companies for billing

Intake forms can be accessed within the Provider Portal by navigating to the Patient Profile. Here, dietitians can view completed intake forms and request additional ROIs.


Patient Welcome Message

Before the initial session, patients are sent an automated email from their dietitian. This message is sent via the messaging feature within the patient's Nourish portal / app. Patients are notified via email of the message, and then can navigate to their Nourish portal / app to view.

The automated message template is provided below for your reference:

Hi {patient first name}!

I wanted to reach out and introduce myself ahead of our first appointment on {date} at {time}. In our first session, we'll work together to understand your health goals and design a personalized approach to reach them. A few quick reminders before we meet:

  • You'll receive a Zoom link for your appointment in an email before the session. Please plan on joining our session from a private location that will allow you to focus on our conversation.

  • If you haven't already, please fill out the intake forms in the Documents section of the app.

  • If you'd like to learn more about your insurance coverage, please reference our coverage calculator on our website.
