Applying for an Oregon State License
Please note: OR is a "green state" so you may see patients here even without an OR state license. A state license, however, is required in order to become in network with BCBS OR (Regence) and UHC OR, which will expand coverage to patients across the country!
If you have held Oregon licensure in the past and would like to renew it, your renewal expenses will be fully reimbursed and you will still receive the $25 bonus!
Step 1: Fill out physical application + payment

The application will ask for ...
1) Basic personal information: name, address, consent to share SSN etc.
2) A checklist of items to include: $110 (check or card, no cash) two forms of ID containing your legal name, one with a photograph, proof of CDR (i.e., pathway 3)
3) Checks can be made payable to "Oregon Health Licensing Office"
Step 2: Mail in the application
Mail in the physical application including payment $110 check (cash not accepted)

Step 3: Upload License to CAQH and Provider Portal
Licensure processing times ~1-2 weeks (please follow up with the Oregon Health Licensing Office if it is taking longer than 3 weeks for your application to process)
Upload it to your Provider Portal when you gain access 2 weeks prior to your start date
Upload it to your CAQH upon approval (in the professional IDs section and the documents section)
Step 4: Reimbursement & Requesting the $25 Bonus
After Oregon processes your license, you will receive a physical copy in the mail. Once you receive your license, you will need to upload a photo of it in both your provider portal and your CAQH (as outlined above). If you applied for the license you are eligible for reimbursement for the cost of license & a $25 bonus!
Submit your reimbursement request (cost of licensure + $25 bonus) via Ramp and include 2 screenshots (a screenshot of your license in the portal and a screenshot of your license in CAQH). Additional instructions on submitting expenses via Ramp can be found here.
Step 5: Credentialing with BCBS OR and UHC OR
Once your license is added to the Provider Portal and CAQH, Nourish will be able to bill both BCBS Oregon (Regence) and United Healthcare Oregon, expanding access to care and improving coverage for your patients!
Note on CEUs:
Oregon requires fifteen (15) continuing education hours every licensure year and one (1) hour of cultural competency continuing education every other. More information can be found here and a list of eligible cultural competency courses can be found here.
