Voluntary Long Term Disability (LTD)

Edited

Long Term Disability insurance provides financial assistance by replacing a portion of your income when you are unable to work for an extended period due to a covered medical condition. This coverage is designed to help you manage your financial obligations while focusing on recovery.

Coverage is available during your initial enrollment period (upon hire or when transferring to a full-time position) without requiring Evidence of Insurability (EOI). If you do not elect LTD coverage during this initial enrollment window and choose to enroll at a later date, you will be required to complete an EOI, and coverage approval is not guaranteed. Once elected during your initial enrollment, you may continue the benefit in the future without needing to complete an EOI.

Please note that our LTD policy is voluntary, and participation is not automatic—you must opt in to take advantage of this benefit.

Highlights of the Long Term Disability Plan:

  • Provider: Our voluntary LTD policy is provided by Unum.

  • Voluntary Enrollment: Participation in the Long Term Disability policy is voluntary. Full-Time employees interested in enrolling must opt in during the open enrollment period or following a qualifying life event.

  • Benefit Amount: 60% of pre-disability earnings, up to $5,000 per month.

  • Cost: $0.176 per $100 covered benefit.

  • When Coverage Begins: After 90 days

  • Maximum Duration: After 90 days of disability to Retirement age

  • Eligibility: This benefit is available to all Full-Time employees.

How to Enroll:

Voluntary LTD offers two enrollment opportunities:

  • At the time of hire or when you become eligible for benefits, you will have 30 days to enroll in the voluntary LTD online in ADP. If you do not enroll during this initial window, you'll have the option to enroll in the next Open Enrollment period, however, you will be required to complete an Evidence of Insurability (EOI) form and coverage is not guaranteed.

  • Please note, any pre-existing condition that existed within 3 months prior to the insured's effective date will not be covered if disability begins during the first 12 months of coverage.
