Update CAQH to reflect practicing with Nourish
CAQH, or the Council for Affordable Quality Healthcare, is a nonprofit organization that aims to improve the efficiency of healthcare by streamlining administrative processes. Nourish, and most other healthcare organizations, utilize CAQH's ProView database, which allows providers to manage and share their credentialing information electronically with payers.
In order to be credentialed with Nourish, you will need to have a completed CAQH profile that has been fully updated to reflect practicing with Nourish. Because payers rely on CAQH data for credentialing, it is absolutely critical that each field of your CAQH profile is correctly updated. Even seemingly small errors can significantly delay the credentialing process, and thus, your start date.
Allow Nourish to update your CAQH profile for you
Because CAQH is time-consuming, complex, and critical, we prefer to update CAQH on behalf of providers. Allowing Nourish to update your CAQH gives you peace of mind that your credentialing applications will process successfully, and saves you time!
Within the Verifiable Onboarding Form (see Verifiable training here), you will be asked to provide consent to Nourish updating your CAQH. A complete list of the updates that Nourish will make on your behalf are listed below.
If you do not wish for Nourish to update CAQH on your behalf, please do not share CAQH credentials in the Verifiable Onboarding Form.

Personal Information:
The following fields will be updated as shown below:
NUCC Grouping: Dietary and Nutritional Service Providers
Provider Type: Dietitian
Practice Setting: Inpatient / Outpatient or Outpatient Only
Primary Practice State: TX (Texas must be listed as your primary practice state for successful credentialing. Doing so generates the Texas-specific CAQH application which is required by our payers.)
Additional Practice States: Every state (other than Texas) in which you hold active licensure will be listed as an additional practice state.
Professional IDs:
The following fields will be updated as shown below:
Professional License: Texas licensure details will be added.
Drug Enforcement Agency (DEA) Registration: The checkbox “I do not prescribe controlled substances” will be marked, with “Reason for not having DEA Registration”, selected as “I don’t have a DEA due to my provider type”.
Specialties:
We will confirm the following:
Primary Specialty: Dietitian, Registered (133V00000X)
Board Certified: Yes
Name of Certifying Board: Commission on Dietetic Registration
Initial Certification Date, expiration date, and last recertification date.
Practice Locations:
Within Practice Locations, we will add two types of practice locations.
A primary practice location for Nourish, Inc that lists our Texas address.
Additional virtual-only practice location(s) for Nourish, Inc [insert state] location, for all non-TX states you hold active licensure in.
Credentialing Contact:
This section is not required because credentialing contacts are designated within each practice location. Any data listed here will override the contact designation within each practice location.
If you have existing data, we will add another credentialing contact in this section for Nourish specifically, rather than deleting existing data.
Professional Liability Insurance:
We will add Nourish's group liability policy to the Professional Liability Insurance section. We will also upload proof of coverage to the Documents section.
Employment Information:
We will confirm that at least five years employment history, with any gaps addressed, are provided. We will also add Nourish as a current employer. CAQH allows you to have multiple current employers, so we will not make any changes to existing data.
Disclosure:
We will confirm all disclosure questions have been answered. We will not update any new disclosure questions on your behalf.

We will update your authorization settings to allow any organization that requests access.

We will review and attest your profile after making any changes to save your data.
What about data Nourish cannot update on my behalf?
If there is any required data that we are unable to update for you, we will notify you via email. We will provide step-by-step instructions on completing the necessary items and will offer support if needed to resolve.
I'm not sure I'm comfortable with Nourish updating my CAQH...
We understand that CAQH contains sensitive information. We take your data privacy seriously. Rest assured, your CAQH credentials will be stored only in Verifiable, a trusted third-party credentialing organization that meets rigorous security standards.
The updates outlined above will need to be made, whether Nourish is making them on your behalf or if you'd prefer to do so yourself. Because the updates are required by payers for successful credentialing, there is no way around making the above updates. We strongly recommend granting Nourish consent to update CAQH on your behalf, as minor errors can cause significant downstream implications on your credentialing, including a denial of your in-network participation and a delayed start date.
If you do not consent to Nourish updating your CAQH, we respect your choice. Please do not provide your CAQH credentials in the Verifiable form. Doing so will ensure that no CAQH updates are made on your behalf.
Frequently Asked Questions:
Q: I provide in-network care outside of Nourish. Will updating my CAQH to reflect practicing with Nourish interrupt coverage for my existing patients outside of Nourish?
A: If your credentialing is already complete with the practice you are currently seeing patients through (meaning, you are able to provide in-network services currently), updating your CAQH to reflect practicing with Nourish will not impact your existing operations or network status outside of Nourish. While we will need you to set your primary practice state to Texas to generate the Texas-specific CAQH application needed for credentialing at Nourish, you are welcome to have other practicing states as additional practice states, and other practice locations as additional practice locations. You can have more than one current employer within the Employment History section of CAQH, so you’re welcome to list Nourish, as well as any existing practices there!
If your credentialing is presently underway outside of Nourish (meaning, you’re in the process of becoming an in-network provider but cannot yet provide in-network care), then it’s possible that updating your CAQH profile to reflect practicing with Nourish could result in delayed or rejected credentialing outside of Nourish. Unfortunately, there is no workaround here, as payers do require your CAQH profile to be fully updated for successful credentialing with Nourish. If this describes your present circumstance, you will need to either:
(1) wait until your credentialing is completed with your current employer, at which point you can update your CAQH data to reflect practicing with Nourish. If you decide to do so, please email onboarding@usenourish.com to align on a delayed Nourish start date.
(2) pause credentialing with your current practice so that your CAQH can be updated to reflect practicing with Nourish, prioritizing your credentialing with Nourish. If you opt for this, you will likely not need to delay your Nourish start date, provided that CAQH updates are made in a timely manner. You can revert your CAQH data to complete credentialing with external practices 30 days after your Nourish credentialing is completed with all payers.
