Overview

The Conditions section in the Provider Portal allows you to capture and validate a patient's primary and secondary conditions in a structured way. This ensures accurate documentation of clinical complexity, supports personalized care, and improves data quality for outcomes tracking.


Navigating the Conditions Section

The dedicated “Conditions” section is in the Assessment & Diagnosis tab of the chart note, directly beneath Patient and Family Medical History.

This section allows you to document the patient’s primary condition and any secondary conditions that may impact the care plan.

When completing this section:

  • Review the patient’s condition list (from the patient's sign-up form) during the initial session

  • Validate the primary condition - update as necessary based on clinical judgment

  • Add all relevant secondary conditions to create a full clinical picture

  • Check the confirmation box once review is complete - this step is required to sign and lock the chart note

The condition list is not visible to patients in the app or portal at this time.


Where to Find It

During a session:

  • Navigate to the Assessment & Diagnosis tab

  • You’ll see a new “Conditions” section with editable fields for primary and secondary conditions

Outside of a session:

  • You can also review or edit a patient’s condition list from their profile view

  • Under the patient’s name and contact info, you’ll see the current condition list and a ✏️ icon

  • Click the icon to make updates


When to Use It

Initial session:

  • This is the most important time to validate the primary condition and ensure the condition list is complete

Follow-up sessions:

  • You can adjust the condition list at any time based on new information

  • No additional prompt will appear, but the list remains editable in both the chart note and patient profile


Why This Matters

  • Improves patient care by helping you recognize comorbidities and tailor your approach

  • Strengthens progress tracking by linking care to condition-specific metrics

  • Enables condition-specific insights by powering dashboards that show outcomes by condition and provider

  • Supports targeted patient outreach by identifying patients who may benefit from condition-specific check-ins, nudges, or follow-up
