Sending Referrals to External Partners

The Nourish Partner Referrals page is a central hub for registered dietitians to refer patients to trusted external providers when care needs fall outside Nourish’s scope.

If a patient requires support beyond what Nourish offers, use this page to connect them with virtual care partners who accept insurance and serve patients nationwide.

To make a referral, click the “Create Referral” button under the appropriate partner. Once submitted, the partner will contact the patient directly to schedule an initial appointment.

Important: Please confirm with the patient that you have their consent before creating a referral for them on this page.
