Step 1: Gather Your Documents

Current CDR Card

  • Password: Create a strong password

⚠️ Password Tip: Your password cannot contain any part of your username/email. Create something unique and secure.

Set Up Multi-Factor Authentication (MFA)

After creating your account, you'll be prompted to set up authentication:

  1. Select "Phone" as your authentication method

  2. Enter your mobile phone number

  3. You'll receive a text message with a verification code

  4. Enter the code to complete setup

💡 Important: You'll need your phone to log in each time, so keep your mobile number up to date.


Step 3: Start Your Application

Log In and Select Application Type

  1. Go to IDFPR-CORE and log in

  2. Enter the verification code sent to your phone

  3. Click "Apply"

  4. In the search box, type "Dietitian"

  5. Select "Dietitian Nutritionist Initial Application" and click "Apply"

Confirm Instructions

Entity Address

  1. Select "Residential Address"

  2. Click "Add"

  3. Enter your current address

  4. If the address doesn't auto-populate, select "Manually Enter"

Email Address

  2. Enter the same email you used to create your account

  3. Click "Save" then "Next"

Professional Certification

  1. Select "Yes" for professional certification

  2. Click "Upload" and select your CDR card

Record of Licensure

Only complete this section if you hold or have held licenses in other states.

  1. Click "Add"

  2. Enter license information for the first state where you were licensed:

    • State name

  4. Click "Save" then "Next"

Original Licensure Question

  • Select "Yes" if you've only ever held one license

  • Select "No" if you've been licensed in multiple states over time
