Application and Fees

To apply for a new license, submit a completed application along with the fee of $50.

You must also submit with your application:

  • Proof of a current, valid Commission on Dietetic Registration (CDR).

Step by Step Application Guide

  1. Complete the Application for Licensure/Certification, checking the box on the form indicating which you want to file for (Dual Licensure/Certification, which allows you to practice as an RD and LD, is the most commonly checked box for Kentucky dietitians).

  2. Enclose the application fee of $50 and a copy of your CDR registration card.

  3. Return everything to State of Kentucky Office of Occupations and Professions, 911 Leawood Drive, Frankfort, KY 40601. Once you are licensed, you may use the LD (Licensed Dietitian) credentials to practice in Kentucky.

Dietitian certifications expire on October 31st of every year.

  • During each year, you must complete at least 15 hours of continuing education.
