Initial Licensing Requirements
Application and Fees
To apply for a new license, submit a completed application along with the fee of $114.
You must also submit with your application:
Proof of a current, valid Commission on Dietetic Registration (CDR).

Download the application.
Complete the application.
Submit completed application and a copy of your CDR via email to dhhs.licensure2117@nebraska.gov.

Step by Step Application Guide
You will receive notice of your renewal with steps for completion at least 30 days before your renewal is due.
To renew your license, you must earn at least 30 hours of approved continuing education within each renewal period.
