Application and Fees

To apply for a new license, submit a completed application along with the fee of $273.

You must also submit with your application:

  • Proof of a current, valid Commission on Dietetic Registration (CDR).

  • Background Check - Detailed Instructions on how to complete the finger printing and background check process here.

Step by Step Application Guide

The Ohio State Licensure process requires 2 parts:

  1. Eligibility: Complete a criminal background check by mailing in your fingerprints (can be conducted at your local law enforcement facility)

  2. Application: Complete the online application. (Note: You must submit your fingerprints to the State of Ohio before you can complete the application)

How to Apply

  1. In order to apply for an Ohio State License, you must first submit your fingerprints to the state of Ohio for a criminal background check (Ohio State Law for any individual applying for a license with the State Medical Board) (~4 weeks to process if you live out of state)

  2. Go to the Ohio Licensing site to create an account

  3. Select “I don’t have a license” under “New Users to the eLicense Portal”

  4. Complete the “Create New Account” steps (First, Last Name, SSN, DoB, e-mail, password)

  5. Once you are in your new account, go to “Dashboard” and click on “+ Apply for a new License”

  6. After final submission (including the finger prints), allow the Ohio State Medical board 10 days to process your application for licensure; you can check your status by logging into the online system and selecting “Options” and “Review Status.” Any outstanding items will be listed in the “Comments” section at the bottom of the page

  • The fingerprinting process

    • Fees to obtain finger prints at your local law enforcement office may apply (TBD, Varies)

    • Personal check or Money Order Check made payable to Treasurer, State of Ohio ($47.25)

    • Priority Mail USPS fees to mail in fingerprints(Varies)

Dietitian certifications expire every year.

Fee: $180

Step by Step Application Guide

  • You can complete your annual license renewal online through the Ohio eLicense portal.

    • It costs $180 to renew your dietitian license in Ohio (plus a $3.50 transaction fee).

    • You must complete one specific continuing education unit

  • You must also renew your CDR registration every five years and show documentation of the completion of at least 75 continuing education credits.
