Oregon is a green state.

However, a license is required for all Nourish RD's that have joined as of 1/1/25 or later.

Nourish will reimburse you for the full cost and provide a $25 bonus after you’ve received your license and have uploaded it within your provider profile in the portal and updated your provider profile in CAQH with the license information.

Application and Fees

To apply for a new license, submit a completed application along with the fee of $110.

You must also submit with your application:

  • Two forms of original identification issued by a government agency

  • Proof of a current, valid Commission on Dietetic Registration (CDR).

    🔹 Applicant Info (Page 1)

    • Include full legal name, DOB, address, phone, email, and SSN

    • Check “No” or list any previous legal names or licenses from other states

    🔹 Payment (Page 1)

    • Mail a check/money order for $110, or

    • Fill out the credit card section and sign it

    🔹 Background Questions (Page 2)

    • Answer Q1–Q3 (disciplinary/criminal history)

    • Sign Section 7 (required)

    • Provider Portal

Have Nearby:

  • Your license information

  • Your PIN

  • Credit card number and expiration date.

You will be guided through the online renewal process. After you have completed your renewal, you will be provided with a confirmation page. Please print the confirmation page as your receipt, which will provide verification of your authorization for 30 days from the renewal date. If you do not receive your permanent authorization to practice by postal mail after 20 days, contact the Health Licensing Office.

If you do not receive your authorization to practice within the 30-day period and the Health Licensing Office has on record that the license was mailed to you, you must submit a written request for a replacement license and pay a $25 fee under Oregon Administrative Rule (OAR) 331-030-0020.

PIN Details

In order to renew your authorization (license, certification or registration) online, you will need your assigned personal identification number (PIN). You must have a valid email address on file with HLO to look up your PIN, as the PIN will be emailed to you. PIN numbers do not change. Please make note of your PIN and keep it for future reference. Follow the steps below to look up your PIN:

  1. Go to: https://elite.hlo.state.or.us/OHLOPublicR/LPRBrowser.aspx to look up your authorization.

  2. Under "Search By Name," enter your last name and first name. If you have a common name, you may need to enter additional identifying information to narrow down the search for your authorization (i.e., license number or type).

  3. Click on the blue "Start Search" button.

  4. Look for your name and the license you would like to renew, and click on the small "arrow" icon to the left of your name.

  5. On the next page, look for "Enter Email Address to Request Renewal PIN Number."

  6. Enter your email address, and click on the blue "Email Renewal PIN."

  7. If you have a valid email address on file with HLO, an email will be sent to your email address with your PIN. Be sure to check your junk or spam folder as well.

NOTE: If you do not receive an email with your PIN, it is likely that you do not have a valid email address on file with the HLO, your email provider has blocked the email from being delivered to you, or your email inbox is full. In this case, please call (503) 378-8667 or email hlo.info@odhsoha.oregon.gov to request your PIN and provide our office with a valid email address for you. Be sure to allow enough time before your renewal expiration date to contact our office and receive a response.

Continuing Education Requirements

Documentation, attestation and audit

Documentation supporting compliance with CE requirements must be maintained by the licensee for a period of two (2) years following license renewal and be provided to the Health Licensing Office upon request. Each licensee shall document compliance with the CE credit requirements through attestation on the license renewal application. Expired Licenses

For each year of inactive status up to three years there will be a $25.00 fee.
