Application and Fees

To apply for a new license, submit a completed application along with the fee of $75.

You must also submit with your application:

  • Proof of a current, valid Commission on Dietetic Registration (CDR).

  • Official Transcripts with completion of Bachelor’s or Masters Degree, from an accredited College or University, with a program in nutrition or dietetics sent directly from the college or university.

  • If you have ever been licensed in another state, license verification(s) must be sent directly from the state(s) in which you hold or have held a license.

Dietitian certifications expire on September 1st of odd years.

Fee: $170

Step by Step Application Guide

  • You can complete your annual license renewal online here.

  • During each two-year licensing period, you must complete 20 hours of continuing education.

    • The state requires attestation of completed credits and individual responsibility for documenting and safeguarding these credits for review by the Board.
