OR License Information - How Oregon Licensing Works When Nourish Applies on Your Behalf

If you received this communication, you’ve been identified as eligible to participate in our Oregon licensing initiative. This effort is optional (but encouraged) and offered at no cost to you.

If you choose to opt in, Nourish will complete and submit your Oregon license application on your behalf. Please review the details below to understand next steps, benefits, and how to claim your $25 bonus once licensed.

Obtaining your Oregon license helps Nourish expand access to care, reduce patient costs, and get patients seen faster in high-demand areas. And we make the process free, simple, and low-effort for you.

Holding a license in Oregon allows you to:

  • Match faster with new patient referrals

  • See more patients in high-demand regions

  • Support Nourish’s mission of improving access across the U.S. (by reducing patient costs and increasing patient coverage)


👥 Who is eligible for Nourish to handle Oregon licensing?

If you received an email from credentialing@usenourish.com asking you to opt in, please complete the form (should take less than 1 minute) and opt in!

If you haven’t received an email and would like Nourish to apply on your behalf, please email credentialing@usenourish.com and let us know you’d like to opt in. We even included an email template you can copy below to speed things up!

  • Template email:

    • Subject line: Please apply on my behalf for Oregon licensure

    • Body:

      Hi Credentialing team,

      I’d love for you to apply for an Oregon license on my behalf. I consent to you using any information from CAQH or my Nourish file (including identity documents) to complete the application, and understand any information used will be kept strictly confidential and only used in the licensing process.

✅ What You Need to Do: Nothing (for now)

We’ll handle the entire Oregon license application and submission process for you.

You don’t need to complete any forms or submit anything up front—we’ll reach out if we need any additional information such as:

  • A copy of your CDR registration

  • Two forms of ID issued as required

  • Any other information needed to support the license application process


📬 After Your License Arrives

Once your license is approved and mailed to you please complete the following steps:

  1. Upload it to:

    • Your Nourish Provider Portal

    • Your CAQH profile under both Documents and Professional IDs

  2. Request your reimbursement + $25 bonus in Ramp

    Be sure to include:

    • Screenshots showing the license uploaded in both systems


⏳ Haven’t Heard Back in 3 Weeks?

Oregon’s board typically processes licenses within 1–2 weeks, but you won’t receive any confirmation.

🙋 FAQs

Q: Will I still receive the $25 bonus if Nourish applies on my behalf?

A: Yes! Once your license is received and uploaded to your CAQH and Provider Portal profiles, submit for reimbursement (bonus) in Ramp.


Q: Do I have to pay the $110 application fee?

A: No - Nourish pays the fee directly on your behalf. You won’t owe anything.


Q: Will this change my day-to-day work at Nourish?

A: No - Nothing will change in your day-to-day. This work is part of the standard licensing and credentialing operations that Nourish handles behind-the-scenes to expand patient access to care.


Q: What are the CEU/Renewal Requirements and Fees?

A: License Renewal required every 1 year

Renewal License Fee is currently $60

Total CEUs Required: 15 hours per 1 year period, 1 hour of cultural competency continuing education every other period.
