How to Renew: Select Renew in the eLicense Ohio Portal
Special Note: All applicants must complete both an Ohio BCI and FBI fingerprint background check and results must be sent to the Ohio State Medical Board.
Application Instructions
Log in to the eLicense Ohio Portal
Create an account if you do not already have one.
From your dashboard, click Apply for a New License.
For License Selection, choose:
Board: Medical Board
License: Licensed Dietitian
Application Type: Reciprocity
Enter your personal, education, employment, and license information as requested.
Upload any required documents directly in the portal.
Review your application and submit payment.
After submission, you will receive an email confirmation.

Background Check & Fingerprinting
All applicants must complete a Combined BCI and FBI Fingerprint Background Check through the FastFingerprints portal.
Go to the FastFingerprints Registration Portal.
Select I Do Not Have a Code.
Select Ohio BCI and/or FBI Background Check.
On the Pick a Transaction Type screen, select Combined BCI and FBI Fingerprint Background Check (do not select BCI-only or FBI-only).
On the Select Service Reason screen:
In the BCI Reason field, enter 4759 061, then click Select for State Medical Board of Ohio – Initial License as a Dietitian.
In the FBI Reason field, enter 4759 061, then click Select for State Medical Board of Ohio – Initial License as a Dietitian.
On the Results Shared Via Direct Copy screen, check the box I would like my results sent to a state agency, then select State Medical Board of Ohio from the dropdown.
Schedule your appointment at a FastFingerprints location.
Pay online — the fee is reimbursable through Ramp.
Results are automatically sent to the State Medical Board of Ohio.
