Overview

Nourish’s Patient Portal is a patient’s one-stop-shop for everything patients need to do before and between sessions at Nourish. It is the only system patients will need to interact with, and is an integral part of their care.

Patients can access the Patient Portal at https://app.usenourish.com. They can also access by visiting the Nourish website and selecting “Sign In.” Patients can also download the Nourish app in the App Store and Google Play Store.

You should ensure your patients have access to the Patient Portal in your first session with them. Encourage them to download the app! It’s important they can access the portal or app to join appointments, set preferences, message you, and access resources, so spend some time making sure they have access. They can always reach out to support@usenourish.com for assistance on downloading our app or signing in.

To sign in, patients should enter the email they signed up for Nourish with, and then enter the most recent code they have received to the email. Patients will receive an automated welcome email from the provider prior to their first session with information on how to access the portal.


Home

The home tab is the default page that patients will see when accessing the Patient Portal. In the home tab, patients can:

  • View their dietitians specialties, styles, and bio

  • Request a new dietitian

  • Join their upcoming appointment

  • View all upcoming appointments and past appointments

  • View claim status on past appointments

  • Cancel, reschedule and schedule future appointments

  • Provide progress updates


Journal

Within the Journal tab, patients can log their meals / snacks! Specific actions include:

  • Logging a meal/snack with text and photos

  • Recording hunger / fullness before and after a meal (1-10)

  • Record satisfaction with a meal (1-10)

  • Write thoughts associated with a meal (free text)

  • Set optional reminders to eat meals or log meals at specific times on specific days

  • See their meal logging “streaks” when they log meals for multiple days in a row

  • View a dietitian’s comments or reaction to a specific meal log

  • Log GI symptoms

  • Edit meal log submissions

There are a few capabilities that we know some patients use that are not included in this first version, but we will be adding in future launches of meal logging (in estimated order):

  • Hydration tracking

  • Eating disorder patient features (urges, feelings, meal setting, meal length, etc.)

  • Stool tracking

  • Macro and micronutrient tracking


Messages

The messaging feature enables providers and patients to message directly in their respective Nourish portals. Patients will reach out between sessions through the messaging feature, so make sure to be periodically monitoring your Provider Portal for any incoming messages! Patients have shown increased likelihood to attend future appointments when they are able to communicate with their providers between sessions.


Food

Modify Health Meal Delivery

Within the Food tab, patients can take advantage of our partnership with Modify Health by purchasing discounted-rate medically tailored meals! For more information on the Modify Health partnership and patient experience, refer to the video demo above and our Modify Health training.

Recipes

Nourish has partnered with recipe content creators to offer 500+ recipes that have been chosen and vetted by our very own Clinical Quality Managers! Recipes are complete with macros / nutritional values, ingredients, and cooking instructions!

You can recommend specific recipes to your patients directly within your Provider Portal. If the recommended recipe matches the patient’s food preferences, it will show up in their “Today’s picks” section (immediately, or the following day if the patient has already reviewed 5 recipes for that day). And, the recommended recipe(s) will also be marked with a “Recommended by” tag on the main recipe detail page, if the patient happens to stumble upon the recipe organically.

Below is an overview of the patient experience when interacting with recipes (and, our Apple Health integration!)


Progress

Within the Progress tab, patients can update their progress and view progress trends. For more information on patient outcomes collection, refer to the video demo above and our Patient Outcomes Collection training.


Profile

Within Profile, patients can view and edit personal information, contact information, and their emergency contact.


Settings

Within Settings, patients can customize notification settings for appointment reminders, message notifications, and reminders to track progress. This is also where patients can sign out of the portal / app.


Documents

Within Documents, patients can view forms completed on intake, complete and sign additional ROIs for external care team members to allow for care coordination, and upload additional documents to share with their dietitian, such as medical records or lab results.


Billing and Insurance

Within Billing and Insurance, patients can view and edit their credit card on file (reserved for collecting no show / late cancellation fees), as well as their insurance information.


Care Team

Within Care Team, patients can view and edit active and archived external care team members. When patients add a new, active care team member, they will be prompted to sign an ROI allowing for care coordination.


Resources

Within Resources, patients can access Nourish-branded resources designed to enhance their nutrition knowledge. Resources cover a variety of topics and are in downloadable formats, allowing the patient to utilize resources the best way they see fit.

Within resources, patients can:

  • Open easily-accessible PDF resources developed by Nourish

  • Filter on specific conditions

  • Search by article title or keyword

  • View resources that providers have specifically suggested they view


Refer a Friend

Patients can utilize the Refer a Friend feature to invite others to join Nourish! The Refer a Friend feature links to this online referral form.


Share Feedback

Patients can share feedback on their experience with Nourish directly within the Patient Portal / App! If patients have urgent questions or issues with their insurance, they are encouraged to contact support@usenourish.com. Otherwise, patients can rate their experience as positive or negative and provide additional feedback as free text.
