How to be eligible for benefits at Nourish
What am I currently eligible for?
In ADP, you will see which plan you are eligible for during Open Enrollment based on your recent weekly occurred averages.
Please log into ADP and navigate to Profile > Work Info > View More > Corporate Groups > Location. Under “Location,” you will see what tier you are eligible for.
Please note, we built in a small buffer so if you are slightly below 15 or 25 weekly occurred averages you may still be eligible, with the expectation that you meet requirements going forward.
If you are not currently eligible for benefits, you can still quickly become eligible for further coverage! Review how to become eligible below.
How can I become eligible for benefits at Nourish?
How do I become eligible for the 25+ tier?
To become eligible for the 25+ tier, you will need to increase your weekly occurred appointments over time and offer sufficient availability.
We understand that ramping does not happen overnight, especially if you are transitioning from other roles. You do not need to already be at a 25 weekly average in order to begin enrollment, but we do need to see a good-faith effort to ramp, which can look like:
Increasing your weekly occurred appointments over time (for example, 5 → 7 → 9 appointments)
Highly encouraged for those ramping up but not necessary if you already have 25+ weekly average:
Allowing same-day or next-day bookings (e.g., minimum initial appointment booking window is 0 or 1)
Allowing 5 or more initials per day when possible (e.g., max initial appointments per day = 5)
Offering at least 30 hours of weekly availability with your availability toggle on (i.e., “available for initials”)
Setting a caseload goal appropriate for 25+ appointments
Once you have done the above steps 1-3, the process is as follows:

How do I become eligible for the 15+ tier?
To become eligible for the 15+ tier, you will need to increase your weekly occurred appointments over time and offer enough availability to support patient access.
We understand that ramping takes time if you are currently very part-time or balancing other roles. You do not need to already be at a 15 weekly average in order to begin enrollment, but we do need to see a good-faith effort, which can look like:
Increasing your weekly occurred appointments over time (for example, 3 → 5 → 7 appointments)
Highly encouraged for those ramping up but not necessary if you already have 15+ weekly average:
Allowing same-day bookings and allowing for flexible booking windows to support new patients
Enabling 3 or more initials per day when possible
Offering at least 20 hours of weekly availability with your availability toggle on
Setting a caseload goal appropriate for 15+ appointments
Once you have done the above steps 1-3, the process is as follows:

I currently have benefits in the 15+ tier, can I switch into 25+ tier?
Yes, you can switch from the 15+ tier to the 25+ tier! You will need to show a good faith effort to ramp up to the 25+ tier (even if you don’t have an average of 25 right away). Please see details above on how to become eligible.
What is the difference between 15-24 eligibility and 25+ eligibility?
Plan design is the same across both tiers. The difference is employer contribution. Those in the Enhanced tier will see increased employer contributions and lower monthly employee costs
Why is the contribution amount different for 15-24 eligibility and 25+ eligibility?
Many RDs shared that stronger benefits would support them in consolidating work and reducing outside jobs. The Enhanced tier reflects that feedback and recognizes the additional workload, schedule stability, and consistency required to support a 25+ caseload
What happens if I can’t maintain the caseload required for eligibility after I’ve enrolled?
To maintain eligibility, you must meet the required weekly occurred average for your tier over time. Eligibility is reviewed and updated on a quarterly basis. If you are unable to meet requirements after a 60 day grace period, your eligibility tier may be adjusted. We will always give you 60 days advanced notice and time to ramp back up.
Your weekly occurred average is calculated as:
Total Occurred Sessions ÷ Total Eligible Weeks Worked
If you begin to consistently fall below your required average:
Nourish will notify you
You will have 60 days to improve and preserve eligibility
If you are still below requirements after 60 days:
Enhanced (25+): you may move to Standard (15+) or part-time eligibility
Standard (15+): you may move to part-time eligibility
Maintaining your average requires:
Adequate availability
Appropriate calendar settings
Patient retention and engagement

Can I do 25+ appointments, but elect into 15+ tier instead?
Yes, you can choose to elect into the 15+ tier even if you currently have a higher average of 25+ patients per week
Can I see 15+ patients per week or 25+ patients per week, but not elect into any benefits?
Yes. Electing into benefits is optional.
How is my weekly average calculated?
(Total Occurred Sessions + PTO) ÷ Total Eligible Weeks Worked
Unpaid Time Off (not formal leave) counts toward total weeks worked
Approved Formal Leave of Absence is excluded from total weeks
PTO taken during leave does not apply toward your average
What about PTO, holidays, sick days, emergencies?
PTO: Can be used to supplement caseload averages (it’s added to your total appointment count)
Unpaid Time Off (not formal leave): Counts toward total weeks worked.
Approved Formal Leave of Absence: Time on leave is excluded from total weeks.
Any PTO taken during an approved leave does not apply toward your session average.
Pauses: If you choose to pause your account, your benefits will be terminated and you’ll be switched to part-time status. Upon returning, you can ramp back up and elect back into benefits if eligible.
Emergencies/ Extenuating circumstances: We understand that things can pop up, and not every week will be perfect; the goal for eligibility monitoring is not to be overly punitive but to ensure you are maintaining the required hours on average and we will take into account extenuating circumstances and company wide trends (e.g., seasonal dips during the holidays when patients do less appointments)
How often can I switch eligibility tiers?
You may upgrade into a higher tier once per calendar year if:
You meet the required weekly average
You meet the availability requirement
You show consistency over a full calendar month
You may move down tiers at any time (to 15+ or part-time), but you cannot move back up again until the following calendar year
What if I took a Nourish-approved leave?
Your average will be calculated excluding any weeks on an approved leave
PTO taken during leave will not apply to your session average
What if I took unpaid time off (not a formal leave)?
Any unpaid time off or informal leaves will be counted toward your total weeks worked while weeks on approved leave are excluded from your average

What if I was eligible n 2025 and have benefits but am no longer eligible?
You will still have benefits until EOY and can ramp back up
How to enroll or waive benefits
I am having trouble logging into ADP. What should I do?
Please try:
Clearing cookies or using a new browser
Confirming you are using: https://workforcenow.adp.com/theme/index.html
Waiting until your employment start date (Open Enrollment access is not available before this)

How do I enroll?
Log into ADP
Navigate to Open Enrollment.
Elect into the plan you are eligible for or waive coverage.
Note: Even if you do not want to enroll, you must formally waive.
